  • have blood taken
  • LABEL THE BLOOD TUBE WITH YOUR NAME AND DATE OF BIRTH, SAMPLE DATE AND TIME
  • mAKE SURE ALL THE DETAILS ARE CORRECTLY COMPLETED ON THE REQUEST FORM
  • PACKAGE THE SAMPLE IN THE MATERIALS PROVIDED
  • mAKE SURE THE REQUST FORM IS INCLUDED IN THE PACKAGE
  • POST TO THE LABORATORY
